Been looking everywhere and have seen all kinds of survey scripts but all I need is a very simple survey form as follows:

Why do you come to our site?

[Radio button] Because it's cool
[Radio button] Stumbled upon it

[Submit button]
(after submission, user will be directed to home page)
---------------------------

I've also tried looking into those form mail scripts but those scripts don't support radio button options.
Ok, maybe they do, but I'm absolutely clueless on how to integrate it.

Anyone?
