It seems that something is adding code to the very top of my pages. Occasionally, something like "<script language=JavaScript src=/ftvhnff.js></script>" is being added infront of my html (that is created with php). The name of the javascript file is random, and always changing. The code is not there all the time, so like if I refresh the same page several times the code will only be added sometimes.

This is causing many 404 errors to be generated. Does anyone know what might be causing this?

(I'm using a Linux server with apache and php)
