form problems--PLEASE HELP!

hello everyone--
i am a newbie to the form world--i am having problems with them--script not executing--sorry i just read that--
here is the url:

the problems:
1.the email it is supposed to go to--doesn't go there
2. supposed to go to a thanks.html page--nope doesn't work either
3. supposed to have 2 required fields--name and phone #
guess what? that doesn't work either!!!

any wisdom, advice i would be your best friend forever!

form values are send to this script: form2mail which is located in /cgi-bin/ folder.

we cant say why it dont work without knowing the code of that perl document.
