Macbeth - they are British uses... at least, if you read Enid Blyton or go to Harvard or Eton... i used to hear that word used in those contexts from some of the Oxford Uni students I knew a few years back...

Ahhh fags are eton specific i believe. Quite famously humphey littleton(famous jazz musician and radio presenter )said on coast to coast US TV he was Lord Harringtons fag when they were at eton together. The show got a fair few letters of complaint so had to annouce what the humphey had meant!
