Submitting Forms

I'm trying ot figure out how to submit a form through Email (the contents of the fields get Emailed). Every time I test it, I get a window that pops up telling me that the recipient will see my Email address, etc. When I hit OK it opens up my Email, but when I receive the Email, it is completely blank. How do I get hte contents into the Email?
