Design the template in the graphics editor of your choice. Slice up the images and code them into html. and upload.

Well you should get a nice graphics program like Adobe Photoshop 5.0+. But if that is too expensive(around $500+), you might want to get Jasc Paint Shop Pro 6 or 7. Then you can read online tutorials at numerous graphic arts sites on the net, or go to your local bookstore and get some nice books. Then with a little time and patience, you'll eventually be able to ake your own. You also might want to get a bunch of freeware filters. Those can always add nice effects.

Asking "how do I make my own template" is basically asking "how do I make a website", since that is what a template it is, a full website design that's just awaiting your custom content.
The answer to this question is time and talent. Creating a "template" requires skill in HTML, graphic design, and a good eye for layout. You've probably seen a good deal of the templates at http://www.freewebtemplates.com These were created by a very talented graphic design artist, and also contains a large number of templates submitted by fans with great talent themselves. Keep browsing, as they update often, and will give you a good feel for a layout done right, and teriffic graphic effects.
